A Dragon dictation alternative for the Mac

Nuance's current Dragon products are built for Windows: Dragon Professional 16 and Dragon Medical One both list only Windows in their system requirements, and Dragon Medical One does its recognition in Nuance's data centers. Loquen is a Mac dictation app for $19 once that runs on the computer's own chip and ships with twelve medical vocabulary packs.

Dragon has been the name in medical dictation for a long time, and Dragon Medical One is deeply woven into hospital systems. If you are a physician on a Mac, though, or you pay for your own tools, the fit is harder. Loquen was made by Nilesh Desai, M.D., a practising internal-medicine physician in New Port Richey, Florida, who dictates notes every day. This page compares the two fairly, including where Dragon is still the right call.

What Dragon runs on

Nuance's release notes for Dragon Professional 16 list Windows 11, Windows 10 and Windows Server 2016, 2019 and 2022 as the supported systems. Microsoft's system requirements for Dragon Medical One list Windows 10, Windows 11 and Windows Server, and the app can also be deployed in Citrix, Microsoft Remote Desktop Services and Omnissa Horizon virtual desktops. Neither document lists macOS.

Dragon Medical One is a cloud service. Microsoft's deployment guide says the speech recognition is performed in secure Nuance data centers, and that the app on your computer does light work such as streaming speech and inserting the results. Loquen does the recognition and the AI cleanup on the Mac itself.

Medical words, on the Mac

General dictation tends to stumble on drug names. Loquen's twelve medical packs, 5,862 terms in all, fix the common mishearings before you see them. Medical core alone has 2,307 terms, built around drug names, generic and brand. The AI cleanup is told not to "correct" pack terms back into ordinary English.

Phrases do the work of templates: say "loquen normal exam" anywhere in a sentence and the whole block pastes, untouched by the AI. Every specialty pack ships a starter set.

Questions

Is there a version of Dragon for the Mac?
Nuance's current documents list Windows only. Dragon Professional 16's release notes name Windows 10, Windows 11 and Windows Server, and Dragon Medical One's system requirements do the same. Loquen is built for the Mac: an Apple Silicon Mac (M1 or later) on macOS 15 or later.
What is a good Dragon Medical One alternative for a doctor on a Mac?
Loquen was made by a practising internal-medicine physician who dictates notes every day. It ships twelve medical vocabulary packs, 5,862 terms built around drug names, and pastes clean text into any app with a text field, an EHR in the browser included. It costs $19 once.
Does Dragon Medical One process speech in the cloud?
Yes. Microsoft's deployment guide says the speech recognition is performed in secure Nuance data centers, and the app on your computer streams speech and inserts the results. Loquen does the recognition on the Mac itself.
What about HIPAA?
Loquen is built for HIPAA the simple way: patient notes never touch a server, so there is no server to trust. HIPAA compliance is about a practice's whole workflow, not one app, so check with your compliance officer.
How do the costs compare?
At September 2026 list prices, Dragon Professional 16 is $700 once and Dragon Medical One is about $1,188 a year ($99 a month). Loquen is $19 once, with a 7-day free trial and a 14-day refund.
