About

Who makes Loquen

Loquen is made by Nilesh Desai, M.D., an internal medicine physician at Family Medical Doctors in New Port Richey, Florida. Dr. Desai dictates patient notes every day, and set out to build a better one after years with the big-name dictation programs.

A physician who charts all day

Dr. Desai trained in Miami and Chicago and practises internal medicine. Most of a clinic day is charting, and much of it happens at the keyboard after the last patient has gone home. Dictation is how that work gets done faster, so over the years Dr. Desai has used nearly every dictation tool that came along: Dragon, MModal Fluency, Wispr Flow, Willow Voice, Eloquent, and half a dozen others.

In Dr. Desai’s own words:

“I watched them get better. Most of them are packed with features. Most of them are also too complicated, or not accurate enough, or not fast enough, or priced like a hospital bought them.”

Why Loquen exists

AI changed what dictation software can do, and there is now a lot of it that saves typing. Almost all of it is either locked to one app, or works everywhere and charges every month, forever. Dr. Desai wanted something different:

The story ends where the product begins:

“So I built it. One key, no subscription, and my notes never leave my computer.”

What that became

Loquen runs speech to text and AI cleanup on the Mac’s own chip. In Private mode, the default, no audio and no text leaves the computer, which is how a physician can dictate a note without sending it to anyone’s server. There is no account, no telemetry, and no screenshots, ever. The details are on the private dictation page.

Because Dr. Desai is a doctor, medicine came first. Twelve medical vocabulary packs ship in the box, 5,862 terms in all, built around drug names and the words each specialty uses, so metoprolol no longer comes out as “metropolis”. In Dr. Desai’s words, “Medicine ships first because that is the job I know.” Packs for law, accounting, engineering and the trades are planned. A pack is one plain JSON file, so anyone can write their own.

It is built for anyone who types, and especially for people who type the same paragraphs all day. Loquen costs $19, once, with a 7-day free trial and a 14-day no-questions refund.

The reason behind it

The motive is a plain one. Charting takes time that would otherwise go to family. In Dr. Desai’s words: “Anything that gives me an hour back with my family is worth building.”

The name

Loquens is Latin for speaking, the root under “eloquent” and “colloquial”. Say it quietly; it still hears you, and it carries no further than your own machine.

Loquen and the practice

Family Medical Doctors has posted the practice’s announcement about Loquen. Loquen is Dr. Desai’s own product, separate from patient care at the practice. Dr. Desai’s profile is on the Family Medical Doctors site.

Made in New Port Richey, Florida, by a doctor who dictates.
