Internal medicine dictation software for Mac

Loquen is internal medicine dictation software for Mac: hold one key, talk, let go, and clean, punctuated text lands at your cursor on your Mac. Its Internal Medicine pack holds 363 terms, so HbA1c, tirzepatide and the Child-Pugh score come out spelled the way you write them.

What the Internal Medicine pack covers

In the pack’s own words: Adult general internal medicine vocabulary spanning endocrine, GI, pulmonary, renal, rheumatologic, infectious disease, hematologic, and geriatric terminology. That is 363 entries. Each one is the spelling you want typed, most with the mishearings a general speech model produces for it. When a term shows up in a dictation, the AI cleanup is told not to “correct” it.

A sample of 29 of the 363 terms, exactly as the pack types them.

What you say, what gets typed

The left column is how a general speech model tends to write the spoken words. The pack swaps it for the right column before the text is pasted.

What you sayWhat gets typed
“h b a one c”HbA1c
“mersa”MRSA
“sem a glue tide”semaglutide
“insulin glar jean”insulin glargine
“mocha”MoCA
“child pew score”Child-Pugh score
“see diff”C. diff
“cue sofa score”qSOFA score

How it fits the day

Internal medicine is the job Loquen was built around: Nilesh Desai, M.D., the internist who makes it, dictates notes every day. Say the problem-based plan, the medication list or the follow-up instructions, and the text pastes at the cursor in any app with a text field, including an EHR in the browser. People speak about 150 words a minute and type about 40.

The pack works alongside Medical core (2,307 terms, built around drug names, generic and brand, plus exam language, labs and imaging) and Pharmacology, which covers the long tail of drug classes. Every pack is included in the price. The packs are English only so far.

The pack also ships a starter set of phrases. A phrase is a named block of text: say its name anywhere in a sentence and the whole block pastes, untouched by the AI. Rename the starters, rewrite them, or add your own.

Private by default

Speech to text and AI cleanup run on the Mac’s own chip. In Private mode, the default, nothing you say leaves the computer: no audio, no text. The initial approximately 2.2 GB model download uses the network but carries no dictation. After setup, licence validation and update checks can use the network and carry no audio or text. No account, no telemetry, no screenshots. Built for HIPAA the simple way: patient notes never touch a server, so there is no server to trust. HIPAA compliance is about a practice’s whole workflow, so check with your compliance officer.
