Psychiatry dictation software for Mac

Loquen is psychiatry dictation software for Mac: hold one key, talk, let go, and clean, punctuated text lands at your cursor on your Mac. Its Psychiatry pack holds 337 terms, so aripiprazole, PHQ-9 and akathisia come out spelled the way you write them, and nothing you say leaves the computer in Private mode.

What the Psychiatry pack covers

In the pack’s own words: Psychiatry vocabulary covering DSM-5-TR diagnoses, psychotropic medications, rating scales, mental status examination language, and psychotherapy modalities. That is 337 entries. Each one is the spelling you want typed, most with the mishearings a general speech model produces for it. When a term shows up in a dictation, the AI cleanup is told not to “correct” it.

A sample of 28 of the 337 terms, exactly as the pack types them.

What you say, what gets typed

The left column is how a general speech model tends to write the spoken words. The pack swaps it for the right column before the text is pasted.

What you sayWhat gets typed
“certain line”sertraline
“airy piprazole”aripiprazole
“close a pine”clozapine
“p h q nine”PHQ-9
“see i w a”CIWA-Ar
“you thymic”euthymic
“acka thisia”akathisia
“skitso phrenia”schizophrenia

How it fits the day

Say the mental status exam, the risk assessment or the medication plan the way you would say it aloud. Loquen pastes at the cursor in any app with a text field, including an EHR in the browser. Esc cancels with nothing pasted.

The pack works alongside Medical core (2,307 terms, built around drug names, generic and brand, plus exam language, labs and imaging) and Pharmacology, which covers the long tail of drug classes. Every pack is included in the price. The packs are English only so far.

The pack also ships a starter set of phrases. A phrase is a named block of text: say its name anywhere in a sentence and the whole block pastes, untouched by the AI. Rename the starters, rewrite them, or add your own.

Privacy first, for psychiatric notes

Speech to text and AI cleanup run on the Mac’s own chip. In Private mode, the default, nothing you say leaves the computer: no audio, no text. The initial approximately 2.2 GB model download uses the network but carries no dictation. After setup, licence validation and update checks can use the network and carry no audio or text. No account, no telemetry, no screenshots. Loquen normally keeps the 50 most recent recordings locally so Retry works; Keep all audio retains more. Built for HIPAA the simple way: patient notes never touch a server, so there is no server to trust. HIPAA compliance is about a practice’s whole workflow, so check with your compliance officer.

Privacy Mode, for text that leaves the chart

Hold a second key and Privacy Mode finds and replaces common identifiers before pasting: names, dates and ages, record and account numbers, phone numbers and emails, addresses. They become plain words, then cleanup rewrites the sentence so it reads naturally. The year of a birth date stays. You see every replacement marked; Return pastes it, Cmd-Z pastes the original. It is built around the categories HIPAA’s Safe Harbor method lists, and it runs on the computer.

A tool, not a compliance programme. Detection is not exhaustive. Review every result before sharing.

An example of what it produces: “The patient, born in 1962, was seen today for chest pain. A family member was notified by phone.”
